Pet Sitter Visit Report Card Template — Free PDF & Editable Download
The report card is the document pet-sitting clients actually wait for — and the one most sitters improvise as a text message. This card makes it a record: arrival and departure times, feeding, the walk, litter, medication marked given, refused or not given, the home and security check, and the photo note — one card per visit, carrying the booking and visit references that tie it to the schedule and the invoice. Paid versions of this document sit behind marketplace paywalls, mostly under personal-use licences; this one is free for your business, printable, editable, and built as the daily link of the FormNest pet-sitting chain.

Example details only — a fictional client, shown so you can see exactly what a completed card looks like. This is visit 1 of Jordan Lee's seven-visit booking with Paws & Paths Pet Care: Scout the dog and Juniper the cat, one 30-minute visit a day, November 9–15, 2026.
| Pet Sitter Visit Report Card — Paws & Paths Pet Care · Visit 1 | |

One card per visit — visits PP-1109-V2 to V7 (November 10–15) each get their own card, never one combined weekly sheet. Seven dated cards are what make the invoice at the end of the booking unarguable.

Frequently asked questions
Should I complete one report card per day?
Yes. Separate dated cards create a reliable record of every visit — and they are what the invoice reconciles against at the end of the booking.
Should I record medication if the pet refuses it?
Yes. Mark refused or not given, note what happened, and notify the owner according to the care instructions. A card that only records successes is not a record.
Can I send the report digitally?
Yes — a completed PDF or a clear photograph of the paper card can be sent by whatever method you agreed with the client. Keep your own copy either way.
