Client Complaint & Resolution Record Template — Free PDF & Editable Download

Most cleaning businesses handle complaints in a message thread — which means the complaint, the promise, the fix and the outcome live in four places and agree with each other nowhere. This record keeps the whole life of a complaint on one document: received, acknowledged, reviewed, resolved, confirmed by the client, and closed with a prevention step so the same problem stops recurring. It pairs with the quality inspection sheet: one catches problems before the client does, the other proves what happened when the client catches one first.

Read the record's discipline: the client's own words are preserved, the acknowledgement took twenty minutes, and the case is only "resolved" after the client said so — not when the return visit was booked.

What's included

Who should use it

How to use it

  1. Start the record the moment a complaint arrives — preserve the client's own wording where you can.
  2. Complete each stage as it happens; never backfill dates from memory at the end.
  3. Distinguish a promised correction from a completed one, and a completed one from a client-confirmed one.
  4. Close every record with a prevention step that has an owner and a date — a complaint that changes nothing will be back.

Printing & customization

Prints on US Letter or A4 across four pages — print a fresh copy for each complaint. Customize the complaint categories in the editable version, and add rows if a case needs multiple visits or follow-ups.

Simple usage licence

Free for your own business use — print it, fill it, edit the editable version and save it as your master. Please don't redistribute, resell or re-upload the files themselves; link to this page instead.

Frequently asked questions

Should I record minor complaints?

Yes. Consistent records are how recurring issues become visible — one missed oven edge is an incident; the same note three times is a checklist problem with a name.

What if the client does not reply?

Record the contact attempts and dates, and leave the case open or mark it closed due to no response, according to your business procedure. What you must not do is mark it 'satisfied' on the client's behalf.

Should I charge for a corrective visit?

Record the agreed arrangement on the form either way. In the worked example, the return visit is provided at no additional charge — the common practice when the miss was the business's.
