Cleaner Training & Skills Sign-Off Sheet Template — Free PDF & Editable Download
Cleaning quality problems are usually training problems that were never written down. This record gives a new cleaner a structured four weeks — orientation and fundamentals, room-by-room practice, equipment and special tasks, then a supervised independent job — with every skill marked honestly as taught, practiced, demonstrated or needing more work. A skill only counts when the trainer has watched the trainee do it to standard. It is the training link of the FormNest chain, and it feeds the inspection sheet: what gets signed off here is what gets checked there.

- Any cleaning business that wants 'trained' to mean something specific and provable
How to use it
- Work through the four weekly sections in order — the structure is a progression, not a menu.
- Mark each skill T, P, D or R as it actually stands; never sign off a skill just because it was explained.
- Use the notes fields for evidence — what was observed, what needs another look.
- Finish with an explicit decision: approved, approved with restrictions, or more training required — and a next assessment date.

Frequently asked questions
Does training have to finish in four weeks?
No — the four weeks are a suggested structure, not a fixed qualification period. Extend the training when a trainee needs more practice; the record's value is that the extension is visible, not hidden.
What does 'D' mean?
Demonstrated: the trainer has observed the trainee perform the skill independently, to the business's required standard. Taught and practiced are steps toward it — only D signs a skill off.
Is a cleaner professionally certified after this?
No. This is an internal business training record. It documents what your business taught and verified — it is not an external certification, and the sheet says so on its face.
